On 13 July 1942, Blenheim V5528 crashed at Manauri, India, when the aircraft hit the ground in a vertical dive and burnt on impact. An Inquiry into the accident found that “the elevator and rudder control cables although intact and correctly locked, were loose.” All on board were killed.
