On 20 December 1939, Warrant Officer Smith was uninjured in the crash of No. 1 Squadron Anson aircraft A4-17 at Pat’s River aerodrome, Tasmania, in which there were no other injuries.

On 28 July 1944, Warrant Officer Smith, a Wireless Maintenance Mechanic, was electrocuted at Milne Bay.
