On 21 January 1943, five members from No. 3 Initial Training School departed on leave in an Army jeep, and an accident occurred at 1345 hours on the Cumbia road about nine miles from Kingaroy. Aircraftman Class II Paynter, an Aircrew Trainee, was admitted to the Kingaroy Base hospital where he died at 1600 hours while Aircraftwoman Ward and Driver Stout were killed and the three other passengers were injured.
