On 13 September 1945, Mustang A65-638 flown by Flight Sergeant Milledge was engaged in a non-operational Battle limb formation training exercise and failed return after the exercise. On 14 September, a civilian report was received that an aircraft was seen diving steeply towards the ground and had disintegrated on impact. A search party located the wreck of A65-638 and the body of Flight Sergeant Milledge 25 miles north east of the Mildura aerodrome.
