On 14 October 1943, Flying Officer McAdam flying Spitfire EN188 was killed in the Benevento area, Italy, while delivering the aircraft to a forward Unit. The weather was poor and there were enemy units in the area at the time. Flying Officer McAdam was an experienced pilot and had completed a number of aircraft deliveries to forward Units.
