During a solo practice flight on 25 March 1941, Tiger Moth A17-137 flown by Leading Aircraftman Mahony crashed at 0730 hours two miles north of Matong, New South Wales, and he was dangerously injured. He was admitted to the Royal Prince Alfred Hospital, Sydney and died of his injuries at 1440 hours on 26 March 1941. The aircraft was apparently flying low and struck trees with the port lower main plane and crashed.
