At 1340 hours on 8 October 1943, Master AZ799 took off from Tern Hill to practice formation flying. The nine aircraft in the formation had an Instructor as First Pilot, and each was carrying a Second Pilot under training. The formation had been flying for 50 minutes, and the aircraft had formed up in echelon port. The Nos. 7 and No 8 aircraft in the formation collided and AZ799, the No. 8 aircraft, crashed killing both pilots.
