MC Citation: For conspicuous gallantry and devotion to duty in maintaining communication between squadrons.  To do this he had to expose himself to exceptionally heavy rifle fire and machine gun fire, and it was owing to his resource in taking and sending message that the regiment was kept together.  (Commonwealth Gazette No. 15 dated 4 February 1919).

On 12 March 1919, Cadet Keen was killed when his aircraft dived through dense low lying mist to ascertain his whereabouts and hit a fallen tree at Misarden Park near Sheepscombe, Gloucestershire.  No further details of the accident are given in his Service Record.
