At 1120 hours on 30 May 1941, Tiger Moth R5186 crashed at Symonds Plains, five miles south east of Western Junction, Tasmania, and Flying Officer Jones was killed. During forced landing instruction practice, R5186 collided with R5185 that was taking off. Both aircraft were restored to service.
