On 28 August 1918, Cadet Jefferys was fatally injured in an aircraft accident at Minchinhampton.  Avro aircraft D/6 in which he was flying solo collided with another aircraft over the airfield during circuit training and both machines crashed out of control.  The name of the pilot of the second aircraft is not given in the record and he was not killed in the accident.
