On 8 November 1941, Beaufighter A19-41 took off on a night cross country training flight on a route from Williamtown – Tamworth – Dubbo and back to Williamtown. The weather forecast some thunderstorms with a cloud base of 1,000 feet and visibility of 10 miles. Four aircraft took off at approximately 2130 to 2330 hours, and as the weather appeared to be deteriorating around Dubbo, the aircraft were ordered to return to base. A19-41 answered the recall signal but due to atmospheric conditions the reply could not be understood. Nothing further was heard from the aircraft and it failed to return to base. A Volunteer Air Observers Corps report in the Gloucester area indicated that there was an aircraft in the vicinity for quite a while about the time A19-41 should have reported to base. Air and ground searches were conducted but no trace of the missing aircraft or crew was found.
