On 16 February 1918, Second Lieutenant Forsyth was fatally injured in the crash of Sopwith Pup aircraft B/6089 at Tern Hill. He was practicing formation flying and, flying into the wind, moved ahead of the formation flying at about 1,000 feet. In an effort to rejoin, he turned back in a vertical bank and the aircraft slipped, the nose dropped and the aircraft went into a spin. He has almost recovered control before hitting the ground. The Court of Enquiry found that the cause of the accident was that Second Lieutenant Forsyth had been overcome by nausea and sickness in the air and lost control of the machine.
