During an air-to-air gunnery exercise 30 kms south of Evans Head airfield on 7 June 1943, gunnery aircraft Fairey Battle L4941 approached towing aircraft Battle L 5650 to give a wash-out signal resulting from a gun stoppage. At 0800 hours the two aircraft collided with L4941 making a forced landing and L5650 crashing and killing its two crew members.
