On 11 May 1918, Second Lieutenant Carr was killed in the crash of Avro Biplane C.619 at Chattis Hill Aerodrome, Stockbridge. A witness reported that the aircraft appeared to be side-slipping out of stall turns, and when at about 200 feet, the aircraft turned on its back, the nose dropped and the aircraft spun into the ground. The Court of Enquiry determined that the accident was due to the temporary loss of aircraft control close to the ground. The Court was unable to determine whether this was due to pilot error or to a mechanical fault.
